Planned Care:
Planned care is usually for those critical or essential tasks that need to be planned in. Remember to follow your home or organisations policy on setting planned care. This is set on Monitor (Computer) and evidenced via the handsets.
You'll be able to identify any planned care with the following flags on the service users profile:

Weight:
On the care app, click into the service user with planned care.
Select the planned care action, this is usually the grey or white tile that is scheduled in.
In this example we will record the weigh.
Click Weigh and you should see the following screen:
To enter the weight click Change.
Now click Weighed
You will either see a graph if you have entered an observation before or a screen saying no graph data. Remember when a height is added to the service users profile (in cm) and the weight recorded in (kg). This will automatically identify the BMI.
Once done add any extra text as required and press Save text.
Complete the rest of the care note as required before completing.
You'll then see the completed care note in the service users profile:
Remember Weighs can also be recorded Adhoc if they are not planned in.
Adhoc Care - Lunch:
For those tasks that do not necessarily need planning in as they could happen at different times are recorded as follows:
Click into the service users profile:
Click Adhoc
Select the icon you are looking for ie Lunch under Nutrition, Eating and Drinking section before pressing Continue.
Use any sliders that are applicable ie Dining location, happiness etc. Remember if you need to add more personalised information then use the change button to add further detail.
Once done enter the time taken and complete the care note.
Now to see it has been recorded click back into the service users profile and you will see the care note. Remember if you need to see further information you can click into the care note like this:
Recording Two handed care - Turn
If any care requires evidencing by multiple staff members ie repositions or 2 handed hoists then it will need recording within 30 minutes of each other by the separate staff.
First staff member -
Click into the service users profile on the handset and select the icon ie Turn
Complete the relevant information and submit the care note
If you click back into the service users profile you'll see it has been recorded:
The one tick at the top right indicates it was recorded by the one staff member.
Second staff member -
Either on a separate device or on the same device once the first staff member has signed out, the second member of staff is now able to log in.
Locate the service users profile.
Click the Repositioned tile done by the first staff member:
This will reveal further details of what was originally recorded:
Click more required and enter your account of what happened
Now complete the care note.
When you click back into the service users profile you will see the following:
There are now 2 ticks. Click into the care note to see further details:
Note staff with access to Monitor will also be able to see any care note recorded, by what staff member and when it was done on the computer / laptop.
Recording Two handed care - 2 Handed Hoist.
When recording a 2 handed hoist you will select the service users profile and either click the action if it is planned in or the Adhoc button.
Collapse the sections at the top right of the screen and select Mobility.
Now click Hoist 2 people and press Continue at the top of the screen
The first staff member can record what they need using the sliders (icons) and use the change button to personalise as needed.
Once done they can Complete the care note.
When you then view the care note by clicking into the service user you will see the following showing one orange tick:
The second staff member then needs to log in and record the action.
Click into the service users profile
Click the care note for the two handed hoist and click do again.
They can then record what they did, again using the sliders if appropriate and utilising the change button to personalise as required.
Once completed, if you go back into the service users profile you will see the following showing two green ticks:
If you click into it, you will see further details.
Note the 2 green ticks on the care note now indicate it has correctly been recorded by the two staff members within the 30 minutes. This will prevent any alerts on Monitor highlighting if it was only recorded by one person.
