Accident & Incident Matrix

The Accidents & Incidents Matrix provides an analysis of all Accidents & Incidents which have been captured over the selected time period. The matrix enables a better understanding of Accidents & Incidents recorded and potential trends in the information being displayed.

The Accidents & Incidents Matrix is beneficial when needing information to:

  • Review and report on trends for a home

  • Use in auditing the number of Accidents, Incidents & Falls which have occurred in the home

  • Highlight any Service Users who are having an increasing number of Accidents & Incidents

To provide further granularity and trend analysis for Accidents & Incidents, various analysis options are available within the Group by and Analyse by filters. The following options are included within the Group by and Analyse by filters.

  • Service User – Each Service user who has had an Accident/Incident during the selected date range will be displayed on the chart

  • Hour – The hour the Accident/Incident has taken place

  • Day of the week – The day of the week the Accident/Incident has taken place

  • Month – The month the Accident/Incident has taken place

  • Accident category – Will indicate the Accident/Incident category recorded

  • Action analysis – Will indicate the Action analysis which has been recorded i.e. First aid given. If no Action analysis has been selected on the form, the information will be grouped or analysed under Not set

  • Resulting in – Will indicate any Resulting in options selected i.e. Hospital admission. If no Resulting in has been selected on the form, the information will be grouped or analysed under Not set

  • Incident intervention – Will display if a care note has been flagged as an Incident and an Incident intervention has been recorded i.e. PRN. If no Incident intervention has been selected on the form, the information will be grouped or analysed under Not set

  • Incident category - Will display if a care note has been flagged as an Incident and an Incident category has been recorded i.e. Medication. If no Incident category has been selected on the form, the information will be grouped or analysed under Not set

The Group by option determines how the information will be displayed on the chart. For example, if grouping by Service User, each Service User’s name will be displayed along the bottom of the chart and their Accidents/Incidents individually counted.

The Analyse by option is used to provide a deeper analysis, such as identifying trends based on the results displayed. For example, if analysing the Accidents/Incidents by day of the week the days each care note recorded during the selected date range, will be displayed along the top of the chart. It is then possible to click on an individual day to hide or show the information you wish to report on.

Each of these options are collated from the Accident & Incident forms, created from the related care notes raised.

Information will only be displayed on the Accident/Incident matrix if the Accident or Incident forms have been marked as Complete.

In addition to the analysis filters, a Falls only checkbox can be selected which when ticked, will only display Accidents & Incidents that have been linked to the Fall action icon.

The benefit of viewing only falls, is to allow for specific examination of falls for each Service User. For example, falls could be analysed separately by Hour and grouped by Service user to understand any trends occurring, which could be used to help prevent future Accidents or Incidents from occurring.

Using this example, it can be seen that numerous falls have occurred at 3:00pm and certain Service Users have had multiple falls at varying times.

The same information can be grouped by Hour and analysed by Service User, to clearly see a trend in the times that Service Users are having the most the falls.

It is also possible to click on an area of the bar chart to see further information on the care note recorded relating to that Accident or Incident.

From this window, you can then click on the Accident/Incident form link to be taken directly to the form linked to that care note.
