The Contingency Plan form captures detailed information for people who have specific and predictable risks associated with their health and wellbeing e.g. diabetes, mental health or substance misuse. The contingency plan provides information such as possible triggers, people who should be contacted should a persons health or circumstances decline and the actions to take. The Contingency Plan is separate to someone’s care plan, but should be informed by it.
1) To create a Contingency Plan form. Click onto the residents profile.
2) On the right hand side of the residents profile click on Care Planning > Assessment Tools.
3) Click on 'Add Form' and then click on 'Contingency Plan'. If you are unable to see this within your list, please discuss this with a manager or a member of staff with 'Admin' access to the system in the home as they may have removed access to using this form.
4) It will be possible to add more than one contingency plan, so the first thing will be to give the contingency plan a name so it is easily identifiable.
The Contingency Plan Contains the Following Fields:
Description of the contingency plan
Trigger factors – clicking Add New will allow you to enter more than one trigger factor up to a total of 5.
What should happen - clicking Add New will allow you to enter more than one set of actions or interventions up to a total of 5.
Who should be contacted? - clicking Add New will allow you to enter more than one contact up to a total of 5. This also has two linked fields for Contact Role and Contact Details.
Coping strategies - clicking Add New will allow you to enter more than one coping strategy up to a total of 5. Each coping strategy can have a start date and an end date in case the strategies change over time.
Relapse indicators/early warning signs - clicking Add New will allow you to enter more than one relapse indicator up to a total of 5.
Does this person have an advance statement – if you click Yes, an additional field will display for:
Date of Advanced Statement and Advance Statement Details
Does the person require any anticipatory medication? – If you click Yes, an additional field will display for 'Detail medicines that may be required in specific situations'.
Was the plan agreed with the relevant person or legitimate representative? – If clicking Yes, an additional field will display for 'Evidence the name, relationship and contacted details of the person(s) consulted'.
If clicking No, an additional field will display for 'Evidence why it has not been practical to get agreement'.
Once the form is saved it will create inform text into a new field called Contingency plans on the current and initial assessment.
Accessing the Contingency Plan on the Care App
It will then be possible to view the Contingency Plan in the care app to allow for quick access to key escalation pathways in the event of a rapid change of circumstances.
1) To access the Contingency Plan on a mobile device, log in to the care app as usual and select any resident. Click on the burger menu on the top right-hand side of the page to display the side-bar menu:
2) Click on the Contingency Plan menu option.
3) A 'Read-Only' version of the different Contingency Plans will then be visible to view.
