How Do I Edit Advanced Care Plan?

The purpose of the Advanced Care Plan form is to document the wishes and plans for 'thinking ahead' as part of the Advance Care Planning discussions.

The information at the top of the assessment is not editable. This information is populated into the form on creation based on information within the resident's 'edit details' page and the 'contacts' set as NOK or to be included within Advanced Care Planning.

The highlighted areas are the areas that you will not be able to edit.

If this information requires updating you will need to archive the assessment by pressing the 'Archive' button in the top right corner.

A new version of the Advanced Care Plan will then need to be created for the Service User. This will then automatically populate the top section of the form with the updated information on the service users profile. Any additional information in the form will need to be manually entered back into the form.

Any information still required can be copied into a Word Document and then pasted into the new version of the form.
