The Independent Icon on the Bowel Movement and Hygiene Charts displays whether the service user is independent for toileting or washing. The icon will appear next to the service users name when within the charts. If you are unable to see this icon appearing then please follow the below instructions:
1) Click onto the Service Users profile.
2) While within the Service Users front profile page, click on Care Planning > Assessment Tools.
3) If you have not yet created a Dependency Assessment for the service user, click on 'Add Assessment' and select 'Dependency'. If you already have one in place, open this assessment in order to make changes to it.
4) For the Hygiene Chart you will need to ensure that you have selected 'Self Care' under the 'Washing' heading. As soon as you save this information in the assessment, the icon will then appear next to the service users name within the Hygiene Chart.
5) For the Bowel Movement Chart you will need to ensure that you have selected 'Self Care' under the 'Toileting' heading.
6) You will also need to ensure that you have selected 'Continent' under the 'Continence' heading. If the service user is not continent in this area then the icon will not display on the charts as they are then classed as not being independent as they require staff assistance.
