Care Story - User Interface Improvements

Provides a chronological view of care delivered to service users across the day. It helps teams understand daily care activities, support continuity of care, and review care delivery over time.

The Care Story page provides a chronological, narrative-style view of care delivered to service users across different times of the day. It brings together care notes into a single, easy-to-read timeline, supporting care, continuity, and review.

Screen 1: Care Story Entry Point

The Care Story page can be accessed from:

Care Story - Deep Dive

The Care Story page displays care notes grouped by service user and organised by date and time of day. When first opened, the page loads in an initial state with no data shown until filters are applied.

Key capabilities include:

  • Viewing care notes across selected date ranges

  • Reviewing care delivered by time of day (Morning, Afternoon, Evening, Night)

  • Accessing detailed care notes linked to each entry

  • Viewing care stories for individual service users or across the service

Initial State (Before Filters Are Applied)

When the Care Story page first loads:

  • The Filters section is displayed at the top of the page

  • No care story data is shown by default

  • An informational message prompts the user to apply filters

What this shows

A message indicating that no information is available until filters are applied.

Use this to:

  • Understand that filters are required to load care stories

  • Avoid reviewing incomplete or unintended results

Screen 3: Filter Section

The Filters section allows you to define which care stories are displayed.

Available filters include:

  • Date From / Date To – select the reporting period

  • Service User – view care stories for all service users or a specific individual

  • Include handover – include handover-related notes

  • Handover Only – limit results to handover notes

  • Separate time of day – group entries by time of day

Once filters are set, select Apply Filters to load the care stories.

Use this to:

  • Focus on specific dates or service users

  • Review handover information where required

  • Tailor the view to suit operational or review needs

Once filters are applied, care stories are displayed by service user and date.

Each entry includes:

  • Date – the day the care was delivered

  • Morning / Afternoon / Evening / Night – narrative care notes recorded during each period

Screen 5: Designated Cell Data Not Available (N/A)

Entries may display N/A where no care was recorded for a specific time of day.

What this shows

A structured, time-based narrative of care delivered throughout the day.

Use this to:

  • Understand daily care patterns

  • Identify gaps or changes in care delivery

  • Support shift handovers and reviews

Viewing Care Notes

Selecting View care notes opens a pop-up modal showing detailed care entries.

This includes:

  • Care descriptions and observations

  • Care category (e.g. Personal Care, Nutrition, Safety)

  • Staff member who recorded the entry

  • Time and duration of the care activity

What this shows

Full contextual detail of care delivered at specific times.

Use this to:

  • Review individual care interactions in detail

  • Support follow-up actions or documentation

  • Provide evidence for audits or investigations

When a specific service user is selected:

  • The page refreshes to show only care stories linked to that individual

  • A View Service User button is displayed for quick navigation to the service user profile

Use this to:

  • Focus on one individual’s care journey

  • Support person-centred reviews

  • Reduce noise when managing larger services

Empty State (No Data Found)

When filters are applied and no matching care stories are found:

  • The Filters section remains visible

  • No care story data is displayed

  • An informational message confirms that no results are available

What this shows

That no care notes exist for the selected criteria.

Use this to:

  • Confirm that no care was recorded in the selected period

  • Adjust filters to broaden or refine results

  • Avoid assuming missing data

Navigation and Page Actions

Navigation controls provide quick access to key actions while viewing Care Story.

These include:

  • Copy API link – copy care story data for sharing or integration

  • Print – print the current view for offline review

  • Return to top – quickly navigate back to the top of the page

Use this to:

  • Share or export care story data

  • Print care narratives for meetings or inspections

  • Navigate efficiently through longer care histories
