The Activities Chart provides a structured view of activities completed by service users over a selected period. It helps teams understand participation, engagement levels, and time spent in activities, supporting wellbeing monitoring, and reviews.

Where to Find The Activities Chart

Screen 1: Care Planning Audit Entry Power

The Activities Chart can be accessed from:

Charts → Care and medical → Activities chart

Activities Chart - Deep Dive

The Activities Chart displays recorded activities based on the filters you apply. When first opened, the chart loads in an initial state with no data shown until filters are selected.

Key capabilities include:

  • Viewing activities by date range and service user

  • Reviewing multiple activity types across selected days

  • Identifying activity status and outcomes

  • Accessing detailed care notes linked to each activity

Initial State (Before Filters Are Applied)

When the Activities Chart first loads:

  • The Filters section is displayed at the top of the page

  • No activity data is shown by default

  • An informational message prompts the user to apply filters

What this shows

A message indicating that no information is available until filters are applied.

Use this to:

  • Understand that filters are required to load results

  • Avoid reviewing incomplete or unintended data

Screen 3: Filters Section

The Filters section allows you to define which activity records are displayed.

Available filters include:

  • Select Month – choose the reporting period (e.g. last 28 days)

  • Select Year – select the reporting year

  • Service User – view activities for all service users or a specific individual

  • Exclude Therapeutic Personal Care – exclude personal care activities from the view (toggle)

Once filters are set, select Apply Filters to load the chart.

Use this to:

  • Focus on specific service users or time periods

  • Exclude activities not relevant to engagement analysis

  • Tailor the view for operational or review needs

Once filters are applied, activities are displayed in a grid layout.

Each row represents an activity type (e.g. Exercise, Gardening).

Each column represents a day within the selected period.

Each cell displays:

  • Activity status icon and label

  • Number of activities recorded for that day

  • A View link to access detailed notes

Statuses may include Done, Not Present, Declined, Refused, or Not Available.

What this shows

A day-by-day overview of activity participation for each service user.

Use this to:

  • Monitor engagement levels over time

  • Identify days with reduced or missed activities

  • Support wellbeing and care planning discussions

Status Indicators and Tooltips

Screen 5: Status Indicators and Tooltips

Explaination:

  • Done - The activity was completed as planned

  • Not Present - The service user was not present at the time the activity was scheduled

  • Declined - The service user chose not to take part (declined) in the activity

  • Refused - The activity was refused by the service user

  • Not Available - The activity could not be delivered due to external factors (e.g. staffing, resources, or environment)

  • N/A (Not Available) - No activity was scheduled or recorded for the service user on that time

Status labels may be truncated to fit within the cell. Hovering over the status displays a tooltip showing the full status description (e.g. Not Present).

What this shows

Clear clarification of activity outcomes where space is limited.

Use this to:

  • Understand activity outcomes accurately

  • Avoid misinterpreting abbreviated labels

Time Spent Outside (Minutes)

Screen 7: Total Time Spent Outside

A summary row displays the total time spent outside, measured in minutes, for each day.

This value is calculated from all relevant recorded activities for the service user.

What this shows The total daily time a service user spent outside.

Use this to:

  • Monitor outdoor engagement

  • Support wellbeing and lifestyle reviews

  • Evidence meaningful activity participation

Viewing Activity Care Notes

Selecting View within a cell opens a care notes pop-up modal.

This includes:

  • Activity description

  • Duration of the activity

  • Outcomes and observations

  • Staff member and time of record

What this shows

Full contextual detail for a specific activity entry.

Use this to:

  • Review individual activity records in detail

  • Support follow-up actions or documentation

  • Provide evidence for audits or inspections

Filtering by Service User

When a specific service user is selected:

  • The chart refreshes to show only activities linked to that individual

  • A View Service User button is displayed for quick navigation

Use this to:

  • Focus on one individual’s activity participation

  • Support person-centred reviews

  • Reduce noise when managing larger services

Empty State (No Data Found)

When filters are applied and no matching activity records are found:

  • The Filters section remains visible

  • No activity data is displayed

  • An informational message confirms that no results are available

What this shows

That no activities were recorded for the selected criteria.

Use this to:

  • Confirm that no data exists for the selected period

  • Adjust filters to broaden or refine results

  • Avoid assuming missing data

Navigation And Page Actions

Screen 11: Navigation and Page Actions

Navigation controls provide quick access to key actions while viewing the Activities Chart.

These include:

  • Copy API link – copy activity data for sharing or integration

  • Print – print the current view for offline review

  • Return to top – quickly navigate back to the top of the page

Use this to:

  • Share or export activity data

  • Print reports for reviews or inspections

  • Navigate efficiently through longer activity histories
