How Do I Make Changes to a Completed Accident/Incident Form?

If you would like to make changes to an accident/incident form that has been marked as 'Completed' you can follow the below instructions to do this. A staff member will sign of a form as being 'Completed' when they have filled out all the required information and it is ready for final sign off by a staff member that has been set up as a Clinical Manager in the home.

In order to reopen a form you must be a Clinical Manager in the home. If you do not have access to this please follow the below guide if you have admin access to the system:

1) Go to Process > Accidents or Process > Incidents.

2) Locate the form that you would like to make changes to. If this has been marked as 'Completed' then the status of the form will appear as 'Pending Signoff'.

3) In the first instance all of the information in the form will be greyed out and not be editable. If you are set up as a Clinical Manager you will see a 'Reopen' box in the top right corner of the form. Clicking this option will allow you to make any changes needed to the form.

4) Once you have made the required changes, scroll to the bottom of the form and press the 'Signoff' button.

Please be aware that once a form has been signed off completely by a Clinical Manager, no further information can then be added.
