Wound Care Plans - Purpose-T

As part of the requirement to evidence on the wound care plan when a specific pressure injury assessment is required, it is now possible to configure which pressure injury tool your organisation uses.

To configure, click on the Admin menu – click Organisation details – click Customisation link. Within this page, there is an option for Pressure injury assessment used.

By default the assessment used will be set to Waterlow. However this can be changed to the Braden scale or Purpose T as needed.

It is also possible to configure the assessment used per care site, in the instance where a home may use a different tool to the rest of the organisation. To do so, click on the Admin menu – click Organisation details – click the relevant care site (home name with a blue background).

By default the Pressure injury assessment used option will be set to Null, which indicates it is taking on the organisation’s setting.

If set to one of the other options not set at the organisation level, the home will display that assessment on the wound care plan.

Once the configuration is saved, the selected pressure injury risk assessment score will display in the wound care plan.
