NOW AVAILABLE - Wound Type, Category and Description Change

An improvement will been made to the Wound Care Plans to allow for changes to the wound types to be made after the record has been saved. This is due to the possibility of errors happening in selecting the appropriate wound type when first creating the wound care plan. A reason for the change will be mandatory when this is changed.

It will also now be required to enter in a reason if the Category of a Pressure Injury is downgraded. This allows for making amendments if a category has been selected in error.​

Alongside this update, the wound description will also been amended to ensure that when a wound has been classified as ‘New’, that classification does not continue to persist throughout multiple reviews.

When a wound care plan has been created and saved, the wound type will now remain editable. This will allow for any corrections or changes to the wound type as needed.

If the wound type is changed, the user will be required to enter a change reason in the Reason for wound type change field which appears. This is to ensure that any changes made are clearly audited. The wound description will also be automatically updated when the change is made.

If the wound is a type of pressure injury and the pressure injury category is downgraded, a pop-up will appear asking the user if they are sure they wish to downgrade the pressure injury

When clicking Yes on this window, it will then ask the user to enter a Reason for category change. If the change was made in error, this can be undone at any point by clicking either No or Cancel depending where the user is on the pop-up.

The Wound summary report has been updated to reflect these changes. There are two additional columns for Wound type changed and Category changed which will show any updates made to these fields and ensure an audit history is maintained.​

Note: if more than one change is made in the same month, the most recent change will display. However, the history is captured on the wound itself.

Within the wound care plan, it is possible to select whether a wound is New or Pre-existing. This is useful information when first creating the wound care plan, however it can be misleading when the terminology persists throughout the entirety of the wound’s treatment.​

For example, the description will hide the word New after the first review of a wound.

To enable better insights into the number of wound type changes that may be occurring within a home, two new group reporting charts have been added to the Safeguarding page of group reporting. ​

Within the Service users with 24 hour care section, there are two new charts for Wound type change and Pressure injury category changes. Both charts highlight when either sections of the wound care plan have been updated and can help to highlight where there may be a lot of changes being made.
