Description
The Choking Assessment (Healthcare Ireland) is a clinical risk assessment designed to identify individuals who may be at risk of choking. The assessment uses a structured set of questions to evaluate risk factors, calculate an overall risk score, and support appropriate care planning, including potential referrals to Speech and Language Therapy (SALT) services where required. The assessment is aligned with Northern Ireland (RQIA) regulatory expectations to support safer care delivery, risk management, and individual wellbeing.

1 | The User must sign in to mCare Monitor. |

2 | The User should select the relevant Community/Sub-Community they wish to access. |

3 | The User must click on the relevant Service User for whom the Choking Risk Assessment will be completed. |

4 | The User should hover over the Assessment/Form dropdown menu and select 'Choking Assessment (Healthcare Ireland)'. |

5 | The assessment consists of 34 Yes/No questions. Each question carries a scoring attribute that contributes to the overall assessment score and corresponding risk interpretation.

6 | Certain responses may prompt a recommendation for referral to Speech and Language Therapy (SALT) services. |

7 | Once all relevant information has been completed, the User will be presented with a final assessment score, including the associated risk range/interval and its meaning. |

8 | Completion of the assessment fields is not mandatory; however, it is recommended that the User answers all questions where possible to ensure assessment accuracy. |

9 | The User may select 'Need Future Review' if required. When selected, the 'Date of Next Review' must be set to a future date. |

10 | Upon successfully saving the assessment, the User will be redirected to the index page, where the assessment name, final score, and score interpretation/meaning will be displayed. |

| You can also view the latest assessment review status directly from the blue tile on the Community page. Clicking on the tile will display the relevant service users who currently require an assessment review. |

11 | The Choking Assessment (Healthcare Ireland) does not currently support Informs functionality. |
|
12 | Any future updates related to this assessment will be communicated by the Product Team. |

13 | Only one active Choking Risk Assessment can exist per Service User at any given time. If the User attempts to create multiple active assessments, a notification will be displayed advising that only one active assessment is permitted. |

14 | To archive an assessment, the User may select the Archive call-to-action (CTA) from the assessment detail page and proceed with the archive process, similar to other assessments within the system. |

15 | Once an assessment has been archived, the User may create a new Choking Risk Assessment (Healthcare Ireland). |
