Description
The Falls Risk Assessment (Healthcare Ireland) is used to assess a Service User’s risk of falling based on key factors such as medication, mobility, medical conditions, sensory deficit, falls history, continence, gender, gait, and age. The assessment calculates a final score and provides a score meaning, while also allowing the User to record action plans, assign responsible persons, set timeframes, and monitor progress.

1 | The User must sign in to mCare Monitor. |

2 | The User should select the relevant Community/Sub-Community they wish to access. |

3 | The User must click on the relevant Service User for whom the Choking Risk Assessment will be completed. |

4 | Before proceeding, the User must ensure that the organisation is operating under RQIA Regulatory requirements, as this assessment is only available for organisations in Northern Ireland or those regulated under RQIA. |

5 | The User should hover over the Assessment/Form dropdown menu and select 'Falls Risk Assessment (Healthcare Ireland)'. |

6 | The User must complete the assessment sections by reviewing the statements related to the Service User’s falls risk. The assessment includes areas such as Medication, Mobility, Medical Conditions, Sensory Deficit, Falls History, Continence, Gender, Gait, and Age.

7 | The User should complete the Summary section by confirming the relevant statements based on the Service User’s circumstances. Where required, the User may add the name of the responsible person and select a time period for completion.
The User may select a completion timeframe such as within 48 hours, enter a specific date, or choose Not specified when the timeframe is not yet confirmed.

8 | The User should use the Action Plan section to record actions or interventions required to reduce the Service User’s falls risk.
To add another action, the User should click “Click here to enter the next step”. The User can then add additional action plan rows as required. Progress can be recorded as Not Done, In Progress, or Done. These action plan updates do not affect the assessment score.

9 | The User should review the Total progress and Overall progress % fields to monitor the completion status of the action plan. |

10 | The User may also tick Needs future review if the Service User’s falls risk requires further monitoring. |

11 | Upon successfully saving the assessment, the User will be redirected to the index page, where the assessment name, final score, and score interpretation/meaning will be displayed.
You can also view the latest assessment review status directly from the blue tile on the Community page. Clicking on the tile will display the relevant service users who currently require an assessment review. , |

12 | The Falls Risk Assessment (Healthcare Ireland) does not currently support Informs functionality. |
|
13 | Any future updates related to this assessment will be communicated by the Product Team. |
|
14 | After saving the assessment, the User will be redirected to the index page. The assessment name, final score, and score meaning/interpretation will be displayed. The score is calculated based on the answers provided in sections 1–9. |

15 | Only one active Falls Risk Assessment (Healthcare Ireland) can exist for a Service User at a time. If the User needs to create a new assessment, the existing active assessment must first be archived. |

16 | To create another Falls Risk Assessment, the User must select the archive option for the existing assessment and complete the archive flow. Once archived, a new Falls Risk Assessment can be created for the Service User. |

17 | Once archived, a new Falls Risk Assessment can be created for the Service User by re-doing step 5-10. |
