NOW AVAILABLE - Purpose-T Update

Written by Rachel Martin

An improvement has been made to Purpose-T Assessment to allow for clinical decision making to be captured. The answers within the Purpose-T Assessment are colour coded and these inform the outcome of the assessment. With the majority of the colour coded answers, it is straight forward what the assessment should be scored at. When there is a mixture of lower risks, there needs to be a clinical decision made and this cannot be programmatically done. When this is the case, the scoring of the assessment will now allow for the outcome to be selected as it was previously read only.

The Assessment Decision section of the Purpose-T Assessment will now be editable when only answers that are blue or yellow have been selected. It will allow the user to select either ‘No pressure ulcer but at risk: Primary prevention pathway’ or ‘No pressure ulcer not currently at risk: Not currently at risk pathway’.

As per the existing functionality, if any of answers selected in the assessment are pink, then the Assessment Decision will automatically select ‘PU Category 1 or above or scarring from previous pressure ulcers: Secondary prevention and treatment pathway’. This answer will be read only and the user cannot select a different outcome.

The same will occur if any of the answers selected are orange. This will automatically select ‘No pressure ulcer but at risk: Primary prevention pathway’. This also cannot be changed.

If the Clinical Judgement is selecting as ‘No problem’, followed by selecting ‘Not currently at risk pathway’, the assessment will automatically be scored as 1 with the meaning of ‘No pressure ulcer, not currently at risk: Not currently at risk pathway’
