The About Me form provides a clearly defined structure to some of the most important details about a person, from preferences, how to communicate with and support someone to general details on their wellness. This form aims to capture the heart of who a person is and what makes them unique, so the focus isn’t just on their health care, but also on their individuality and wellbeing.
To make the information captured on this form more easily accessible, a quick link to the form has been added on the main page of a resident’s profile.
The form has also been added on the Care App, so all carers can read this key information at the point of care. This will be especially important for new staff or agency workers who do not know the person as well and wish to understand more about how they want to be treated.
1) To access the 'About Me' profile on a mobile device, log in to the care app as usual and select any resident. Click on the burger menu on the top right-hand side of the page to display the side-bar menu:
2) Click on the option from the drop down called 'About Me'.
3) Once clicking on the above option a 'Read-Only' version of the About Me profile will appear for the service user.
